Put on a serious expression, push your lower lip up slightly, and use your index finger to pretend to poke an imaginary hole in your cheek.

Furbo!

Furbo - smart: With an approving look on your face, use your thumb to trace a line across your cheek.

The undisputed classic of all Italian hand gestures: one that could be described in a PG-rated way as "WTF!" or something like that. When words fail, this can also be accompanied by a barked "Ouw!".
